The Foundation Services portion of our site is designed to assist Santa Rosa Junior College faculty and staff with the day-to-day aspects of working with Foundation accounts.
Contact Report Form is to be completed when a board member or employee makes contact with an individual who may be interested in the Foundation.
Endowment Agreement Form (see sample) is filled out to establish an endowment fund and requires an initial balance of $10,000 or more. The Criteria & History Form is used in conjunction with this form. Contact staff to assist with the completion of this form.
Gift-In-Kind Form is for non-cash gifts in accordance with Foundation gift acceptance policy.
Information Request Form is used to formally request information regarding a fund balance.
Media Services Release Form, once completed, allows the Foundation to use images in publications.
Pledge Agreement Form is to be completed and signed to formalize a pledge to a Foundation managed fund.
Request for Disbursement Form is for accessing funds from Foundation managed accounts. Contact staff to assist with the completion of this form.
Voluntary Payroll Deduction Form is completed by employees who wish to make gifts through payroll deduction.
